- For Ages 51-60: Non-Med $200-$3,000 / Abbreviated Paramed, Blood Profile, Urinalysis: $3,001+
Changes to Financial Documentation Guidelines for Self-Employed and Business Owners
- One year of income documentation is required for benefit amounts between $3,001 and $5,000 per month (including Integrated Monthly Benefit Rider and existing coverage, all sources).
- Two years of income documentation is required for benefit amounts greater than $5,000 per month (including Integrated Monthly Benefit Rider and existing coverage, all sources).
